Start by navigating to the Quizzes tab inside the Sites section and editing your desired build.
Scroll to the bottom of the builder and click Edit Template.
Click the dial icon in the top-right corner to access settings.
Toggle the button next to the values you want to create a Custom Field for, then click Save.
